Health and Safety Accident and Incident Investigations, setting up safety investigations to ensure all required control measures are in place, and if not, why not, also looking at why the accident occurred and not apportioning blame to any one individual, basically are the safety systems (e.g. Safe Systems of Work, Method Statements, Permit-to-Work, Risk Assessments), which are in place, working, and are they being used correctly; and

happened and can identify whether further controls are required, to ensure, as far as reasonably practicable, the accident will not happen again.  An Accident Investigation can provide information on why and how existing controls were breached.  We can also help employers identify where they may be missing out on certain clues on improving their accident/incident records, by addressing the problem of Near Miss/Non-injury Accidents etc.   Certain workplace accidents/incidents, diseases, injuries, illnesses and dangerous occurrences must be reported to the enforcing authorities, in the UK these would be reported through the Reporting of Injuries, Disease's and Dangerous Occurrences regulations (RIDDOR)
